Reopening note. Three controls this review listed as follow-up work are not in main.

Shape validation of every provider response, the https-endpoint requirement, and the
session-cookie payload check after the MAC check. src/lib/auth/oidc-schemas.ts and
src/lib/env.ts do not exist on main.

PR #216 is that follow-up. It also fixes three fail-open defects the validation exposed, including
a non-finite exp that never expired.
